We understand that chronic (“ongoing”) diseases affect not just physical health but emotional wellbeing, daily routine and quality of life and our local GPs, nurses, and allied health providers are here to help every step of the way.

What is Chronic Disease Management

Chronic disease management refers to a holistic, planned approach to treating illnesses that last six months or more (and often much longer), such as:

  • Diabetes

  • Cardiovascular disease (heart disease, hypertension)

  • Chronic obstructive pulmonary disease (COPD) / respiratory conditions

  • Asthma

  • Stroke recovery

  • Musculoskeletal conditions

  • Cancer‑related long‑term effects

These conditions often require ongoing monitoring, medication adjustments, lifestyle changes, regular check‑ups, health education, and sometimes working with multiple health professionals.

Our Chronic Disease Care Process

Our chronic disease care is not just about prescribing medicine, it’s about working together to manage, prevent complications, and improve quality of life. Here’s how:

  1. Initial Assessment
During your first appointment, we take a detailed medical history, assess your current health status, risk factors (diet, exercise, smoking, family history), and any complications.
  2. Personalised Care Plan
Your GP will collaborate with you to prepare a Chronic Disease Management Plan tailored to your health goals. This includes medication review, lifestyle changes (nutrition, activity, sleep), monitoring schedules, and possibly referrals to specialists or allied health (dietitian, physiotherapist, podiatrist etc.).
  3. Lifestyle & Preventative Care
Preventative care is central. Identifying risk factors early, encouraging healthy living, regular screening (e.g. for cardiovascular risk, lung function), immunisations, and education so that you are empowered to take control over aspects of your health.
  4. Ongoing Review and Monitoring
We schedule regular follow‑ups to check progress, adjust treatment, and monitor for any new issues. If needed, medication may be tweaked; further tests may be ordered; allied health referrals arranged.

  5.  Support & Coordination Where multiple providers are involved (e.g. specialists, allied health), we coordinate care so it remains unified, seamless and centred around your needs. We also help with navigating resources, appointments, and ensuring care is realistic for your life in Casuarina.
